# Why are questionnaires required for the Initial Consultation?

The purpose of the Initial Consultation is for the LifestyleRx physician to get to know you better through a review of your medical history and Wellness Questionnaire. Without the completed questionnaire, there would be very little information to review during the consultation.

In addition, the questionnaires are required in order for you to receive your Personal Health Plan (Report) and an invitation to book your Report Review appointment.
