# Program Eligibility

To immediately qualify for the program, patients must have a medical diagnosis of one or more of the conditions listed on the LifestyleRx website under “All Programs”:

Patients may still be able to enrol in the program in consultation with their physician. You can schedule an appointment with your doctor to review the program. If your physician feels it is appropriate and that you qualify, they can send a referral to us to get you started.
